Beskrivning | Aim: To probe into effect of innocuous stimulation induced by chiropractic and massage on treating diabetic gastroparesis (DGP). Methods: Totally 106 type 2 DGP patients were selected from Endocrinological Department and Metabolic Department of Central Hospital of Jiaozuo Coal Limited-liability Company from January to December 2004. All patients were willing to test. All 106 patients were randomly divided into observational group (n=53, 17 males and 36 females) and control group (53, 19 males and 34 females). Patients in the two group were interfered on the basis of controlling blood glucose as followed: control group: patients were taken orally with mosapride (commercial name: Bellona, provided by Kanghong Pharmaceutical Group, batch number: 200401) 5 mg/time, three times a day, 7 days as a course, and 4 course in total; observational group: Patients were treated with chiropractic and massage, 7 days as a course, and 4 courses in total. The details were as followed: chiropractic: from Changqiang, through Dumai, to Chuixue; main points of symptom such as spleen, stomach, liver and geyu for twice. Six cycles were, regarded as one course for two or three times in total. Massage was performed after chiropractic. Totally 34 points from top to bottom were massaged from cervical part to vertebra with 300 times or increase and decrease times according to severe degrees. The massage was performed once respectively in the morning and in the afternoon for 30-40 minutes. 2 Clinical effective criteria: Inflation, early fullness, nausea, emesis, anorexia and belch of stomach were classified into five degrees: grade 0: symptomless; grade 1: occasional symptom or mild level, non-effect on living and work; grade 2: intervenient grade 1 and grade 3; grade 3: frequent symptoms, obvious effect on living and work; grade 4: severe symptoms, great distress in patients. Each grade was scored as 0, 1, 2, 3 and 4 points to calculate total value and effective index before and after treatment [(scores before treatment-scores after treatment) scores before treatment x 100%]. Effective index > 75% was regarded as apparent effect, 51%-75% as effect, 36%-50% as improvement, and < 26% as no effect. 3 Fasting blood glucose and 2-hour blood glucose were assayed before and after treatment. 4 Duration of migrating motor complex (MMC) I, II and III in stomach sinus and duodenum at interdigestive phase and intensity of systolic wave were measured with PC POLY-GRAF HR gastrointestinal functional monitor (CTD-SYNECTICS Company, Sweden) and gastrointestinal manometry. Electron main frequency and electron main duty of stomach were assayed at the same time. 5 Enumeration data and measurement data were compared with x2 test and t test, respectively. Results: Totally 106 type 2 DGP patients entered the final analysis. 1 Effect in observational group was greater than that in control group (P < 0.01). 2 There was not significant difference between fasting blood glucose and 2-hour blood glucose in control group before and after treatment (P > 0.05). Fasting blood glucose in observational group was not significant before and after treatment (P > 0.05), but 2-hour fasting blood glucose was decreased obviously (P < 0.05). 3 Duration of MMC-III in stomach sinus and duodenum at interdigestive phase was longer after treatment than that before treatment in the two groups (P < 0.05); intensity of systolic wave-III was increased after treatment as compared with that before treatment (P < 0.05); during of MMC-II was shorter after treatment than that before treatment (P < 0.05); duration of MMC- I was not significant before and after treatment (P > 0.05). During of MMC- I, II and III was shorter in observational group than that in control group after treatment (P < 0.05), and intensity of systolic wave-III was stronger in observational group than that in control group after treatment (P < 0.05). 4 Electron main frequency and electron main duty of stomach were higher after treatment than those before treatment in the two groups (P < 0.05-0.01). 5 Various side effects were found in control group, but not in observational group. Conclusion: Chiropractic and massage have both whole and local therapeutical effect, and can promote recovery of stomach motive force and absorption of drugs to decrease content of blood glucose, and side effects are not found out.
